All individuals 16 years of age and older that reside in the United States are eligible to be vaccinated at NYC Health + Hospitals.
NOTE: Individuals being vaccinated must produce proof of eligibility at time of appointment. Please also bring your health insurance card if you have one. You will not need to provide proof of immigration status or a social security number to get vaccinated.
Proof of Age may include one of the following: Driver’s license or non-driver ID, a birth certificate issued by a state or local government, Consulate ID, a current U.S passport or valid foreign passport, a permanent resident card, a certificate of Naturalization or Citizenship, a life insurance policy with birthdate or a marriage certificate with birthdate, or any legal proof of your date of birth and residency.
Minors must have identification to verify they are at least 16 years of age. A parent or legal guardian must also provide consent for vaccination at the vaccine appointment.

Only the Pfizer vaccine is approved for those 16 or 17 years old.
