NYC Health + Hospitals Emergency Hotel Employee Program serves as place of isolation for non COVID employees in active status whose work puts them in direct contact with COVID patients. Initial stays of up to 14 days will be granted.

This form is for new reservation requests only. To request an extension on an existing stay, please select “Hotel Reservation Extension Request” from the drop down button located above.
If you are currently enrolled in the staff hotel program and interested in requesting an extension of stay, you must complete this online form at least five (5) business days prior to your scheduled check out date. Extension approvals are at the discretion of NYC Health + Hospitals and not guaranteed. Employees may request up to 2 extension requests. Extension stays have an 8-day limit per each extension request.
A hotel room can only be reserved for isolation purposes.
Requests will be reviewed by Human Resources to verify your employment status, and determine the appropriateness of the request and length of stay.
If you do not receive approval confirmation prior to the date of your scheduled departure, you are required to check out on your scheduled check out date. Any personal belongings left-behind will be subject to the rules and policies set forth by the hotel.
